MIHS Online Membership Submission Form Instructions (Pay Pal)
The MIHS Online
Membership Form, upon completion and submittal, will send you to
a page with Pay Pal links to all memberships levels. Use these links
for both new memberships and renewal memberships.
New Pay Pal
members, please have the following information available before
continuing:

must have your credit card number, expiration date, account holder
name, and billing address available.
If you are using
a Corporate card, please confirm this information with you financial

your account number, branch address, routing number and follow the
instructions for verification. |

This must
be a combination of your first name's initial and your last name,
in lower case letters.
i.e. John Smith

This can be any
combination of letters or numbers that you can readily remember. You
will be asked for this information when you login to your MIHS account. |
E-mail
Address: |
Enter the
E-mail address you want to receive information regarding your MIHS
membership, which may include newsletters, meeting notices and membership
updates.
If by chance,
you do not have an E-mail address, please enter the word "none"
followed by the @ symbol
